Throat replacement again

Well here we are again the in the Repair cycle of our 3D Printer.
Filament enters at 1.77mm expands inside throat to 2.21mm and gets stuck.
The PTFE Throat has been enlarged to 2.21mm inside the bottom of the throat and pushed into the Nozzle narrowing the channel in the nozzle.
Throat on the left, nozzle on the right. both sitting in T8-8 lead screw nuts. (Stupid Digital camera can't correctly focus this auto focus camera.)
This happens about 3 times a year. And in the process messes up about 3 or 4 printer or until if discover the throat PTFE tubing is bad again.
